CO-4 denial: Modifier issue
The payer says the procedure code is inconsistent with the modifier on the claim, or a modifier the code needs is missing.
Common causes
- A required modifier was left off the line
- A modifier was added that does not apply to that procedure code
- Modifier order or placement does not meet the payer's edit
How to fix it
- Check the original claim line against the chart and the payer's modifier policy
- Correct the modifier and submit a corrected claim, not an appeal
- If the modifier was right, call the payer and ask which edit fired
How to prevent it
- Run claims through a scrubber with current modifier edits
- Keep a payer-specific modifier cheat sheet for your common services
Zenith billers and AR specialists work denials like this inside your practice-management system, and your practice stays the billing provider. See AR follow-up and medical billing.
