Zenith Care Solutions

Principal Care Management for one serious condition.

Principal care management (PCM) is a monthly Medicare service for a patient with one serious, high-risk condition expected to last at least three months, the kind that can send them to the hospital if it slips. Zenith clinical staff deliver the monthly outreach, care plan reviews and time log inside your EHR (CPT 99426 and 99427), so that one condition gets steady attention between visits. Your practice stays the billing provider.
