Zenith Care Solutions

Transitional Care Management that closes the gap after discharge.

The days after a hospital stay are when patients are most likely to be readmitted. TCM has strict deadlines, and missing them means missing both the care and the claim. Zenith staff make the post-discharge contact on time, reconcile medications and get the follow-up visit booked. This is a staffed service, not software: our team works inside the tools you already use. Your practice stays the billing provider.
