CO-11 denial: Diagnosis mismatch
The payer says the diagnosis code on the claim does not support the procedure billed.
Common causes
- The diagnosis pointer on the claim line points to the wrong code
- The diagnosis is valid but not one the payer accepts for that service
- A coding or documentation gap left out the supporting diagnosis
How to fix it
- Compare the chart, the diagnosis codes and the line-level pointers
- Correct the pointer or add the supported diagnosis, then submit a corrected claim
- If the coding is right, appeal with notes showing the medical reason
How to prevent it
- Check payer coverage lists for diagnosis and procedure pairs before billing
- Ask providers to document the reason for each service clearly
Zenith billers and AR specialists work denials like this inside your practice-management system, and your practice stays the billing provider. See AR follow-up and medical billing.
