CO-50 denial: Medical necessity
The payer decided the service was not medically necessary under its coverage policy.
Common causes
- Documentation does not show why the service was needed
- The diagnosis does not meet the payer's coverage policy for that service
- The service was done more often than the policy allows
How to fix it
- Read the payer's coverage policy for the service and compare it to the chart
- Appeal with the visit notes, test results and a short letter tying them to the policy
- Follow the payer's appeal deadline; Medicare redetermination requests are generally due within 120 days of receiving the remittance (receipt is presumed 5 days after its date)
How to prevent it
- Check coverage policies for high-cost or frequent services before they are done
- Train providers to document the reason, not only the result
Zenith billers and AR specialists work denials like this inside your practice-management system, and your practice stays the billing provider. See AR follow-up and medical billing.
