CPT 99439 vs. 99490: How the Add-On Code Works and When to Bill It

Most practices that launch Chronic Care Management are familiar with CPT 99490, the base code for the first 20 minutes of monthly care coordination. Fewer consistently capture CPT 99439, the add-on code that pays for each additional 20-minute increment. This guide explains both codes, when you can bill them together, and what documentation you need for each.

What does CPT 99490 cover?

CPT 99490 is the base CCM code. It covers the first 20 minutes of qualifying clinical staff time spent on chronic care management activities in a single calendar month. The time must be:

  • Non-face-to-face (not counting office visit time)
  • Performed by clinical staff under general physician supervision
  • Directly related to care coordination for the patient's chronic conditions
  • Documented in the patient's medical record with a time record

The 2026 national average reimbursement for 99490 is approximately $66 per enrolled patient per month. Actual payment varies by Medicare locality.

For the full breakdown of eligibility, consent requirements, and how to submit the claim, see the CPT 99490 billing guide.

What does CPT 99439 cover?

CPT 99439 is an add-on code. It covers each additional 20-minute increment of qualifying clinical staff time beyond the first 20 minutes already captured by 99490. It cannot be billed without the base code 99490 in the same calendar month.

Key rules:

  • CPT 99439 may be billed up to twice per calendar month (a maximum of 40 additional minutes, for a total of 60 minutes under 99490 + 2x 99439)
  • Each unit of 99439 requires its own documented 20-minute increment
  • Time must be continuous or accretive over the calendar month (it does not need to occur in a single session)
  • The same documentation standards as 99490 apply: date, activity, duration, and the staff member's name in the time log

The 2026 national average reimbursement for 99439 is approximately $46 per unit. With two units billed, that is an additional $92 on top of the $66 base.

Can you bill both 99490 and 99439 in the same month?

Yes, and for high-need patients this is the expected pattern. The combination unlocks significantly higher monthly revenue per patient:

| Codes billed | Total documented time | Approx. 2026 payment (national avg.) | |---|---|---| | 99490 only | 20+ minutes | ~$66 | | 99490 + 99439 x1 | 40+ minutes | ~$112 | | 99490 + 99439 x2 | 60+ minutes | ~$158 |

All figures are approximate 2026 national averages and vary by Medicare locality.

The most important rule: do not bill 99439 unless the time has been documented. A common billing error is to add 99439 units preemptively or as a blanket practice. The time must be in the record before the claim is submitted.

What does the 2026 reimbursement look like compared to complex CCM?

For patients whose conditions are significantly complex and require substantial care plan revision by a physician or qualified non-physician practitioner, the complex CCM codes pay higher rates:

| Code | Description | Approx. 2026 rate | |---|---|---| | 99487 | Complex CCM, 60+ min, physician or QNP involvement | ~$134/month | | 99489 | Complex CCM add-on, each additional 30 min | ~$70/unit | | 99490 | Standard CCM base, first 20 min clinical staff | ~$66/month | | 99439 | Standard CCM add-on, each additional 20 min (up to 2x) | ~$46/unit |

Note that 99487 (complex CCM base) and 99490 (standard CCM base) are mutually exclusive in the same calendar month for the same patient. A patient is in one program or the other, not both.

What documentation is required for each code?

For CPT 99490:

  • Documented patient consent (obtained before first billing month)
  • A written care plan in the EHR
  • At least 20 minutes of qualifying clinical staff time, logged with date, activity, and duration
  • The billing provider's oversight relationship noted (incident-to supervision)

For each unit of CPT 99439:

  • An additional 20 minutes of qualifying time logged in the same calendar month
  • The same activity-level documentation standard as 99490: date, activity type, staff member, duration
  • A clear running time total in the documentation that shows the cumulative minutes for the month

The running time total matters for audits. Auditors reviewing a claim with 99490 + 99439 x2 will expect to find 60 minutes of documented qualifying activity in that calendar month. If the logs show 35 minutes, the two 99439 units will be denied and the overpayment recovered.

When should you escalate to complex CCM codes instead?

Consider escalating to 99487/99489 when:

  • The patient's conditions have become significantly more complex (new cancer diagnosis, multi-organ involvement, medication regimen requiring frequent physician-level adjustment)
  • A physician or qualified non-physician practitioner is the one providing the care management, rather than delegating to clinical staff
  • The care plan requires substantial revision in the month, not just routine updates
  • 60 or more minutes of time is consistently being logged and the physician is directly involved in a meaningful portion

The documentation threshold for complex CCM is higher because a physician or QNP must be personally involved. If the work is primarily clinical staff calling the patient monthly, standard CCM (99490/99439) is the appropriate code family.
